Development, testing and production environments are separated and secured.
What an assessor looks for
Show the separation technically and in terms of access. Shared credentials or an identity crossing all three undermine the control however the infrastructure is arranged.
Where it usually fails
Environments are separate while the same administrators and the same credentials span all of them.
