Events are assessed and a decision is taken on whether each is an incident.
What an assessor looks for
Show the triage criteria and a sample of events assessed, including ones judged not to be incidents. The discarded events demonstrate the control as clearly as the escalated ones.
Where it usually fails
Only confirmed incidents are recorded, so there is no evidence that assessment happens at all.
