What is inside
An Excel workbook of 18 questions across nine areas: model provenance, training data, tenancy and isolation, human review, security of the AI layer, accuracy and failure, governance, exit, and incident terms.
Each question carries why it matters, what a good answer looks like, and, in red, what should worry you. Several of these questions have a confident-sounding answer that is actually a red flag, and those are named. The vendor fills two columns; you score the answers yourself, and the Scoring tab totals by area so you can see where the risk concentrates.
